Job summary

Phoenix Futures seeks a dedicated practitioner to support Gypsy, Roma and Traveller communities across Essex. You will work in a small team, holding a caseload and delivering outreach, advocacy and referrals within a multi-agency network.

Qualifications

• Experience in health and social care or related field
• Ability to work with partner agencies for client benefit
• Caseload management and strong organisational skills
• Proficiency with Microsoft Outlook and Word; familiarity with databases
• Excellent written and verbal communication
• Driven, compassionate and learner
• Passionate about improving lives of Traveller communities

Responsibilities

• Deliver a service to Traveller communities across Essex (excluding Southend and Thurrock)
• Provide outreach, advocacy, signposting and onward referrals
• Build links with local partners to form a multi-disciplinary support network
• Manage a caseload and accompany clients to appointments as needed
• Work in a collaborative, multi-agency environment to empower clients to access services
