Job summary

Phoenix Futures Group is recruiting a Practitioner to join the Enhanced Support and Access Team, based in Chelmsford. You will hold a caseload and work intensively with a multi-agency network to empower Traveller communities to access essential services and sustain positive change.

The role requires experience in health and social care, strong organisational skills, and excellent communication. We offer a Real Living Wage, generous leave, pension, and ongoing training.

Qualifications

• Experience in health and social care or comparable field.
• Ability to work effectively with partner agencies and network for the benefit of the client and service.
• Ability to manage a caseload and excellent organisational skills.
• Proficient in Microsoft Outlook and Word and familiar with database management systems.
• Excellent communication skills, both written and verbal.
• Determined with a drive to succeed and a willingness to learn.
• Passionate and enthusiastic about making a real difference to the lives of people we support.

Responsibilities

• Hold a caseload and work in an intensive, multi-agency way to empower people within the Traveller community to access a wide range of services.
• Outreach, advocacy, signposting and onward referrals (including accompanying people to appointments).
• Forge strong links with local partners and services to form a multi-disciplinary network of support.
