Job summary

Wye Valley NHS Trust is recruiting a Physiotherapy Assistant Practitioner (Band 4) for an inpatient acute therapy team at Hereford County Hospital. The role involves assessing patients, delivering physiotherapy interventions, and planning discharges with specialized equipment where needed.

The post is a 6-month fixed-term/secondment with 37.5 hours per week, salary £28,392–£31,157 per annum pro rata, and a commitment to multi-disciplinary care and service improvement.

Qualifications

• Foundation degree or Level 5 in Health & Social Care or equivalent.
• Hold a Care Certificate or Level 5 Apprenticeship route as alternative.
• Clinical experience within AHP with theoretical understanding.

Responsibilities

• Assess and treat patients with acute needs on the ward.
• Plan discharge with patient-centered goals and equipment where needed.
• Provide exercise programmes and assist rehabilitation.
• Work within a multidisciplinary team to support patients.
