Job summary

Radis Community Care is seeking an Extra Care Team Leader to support residents living in their own homes within the Portishead extra care complex. You will lead a team to deliver person-centred care and maintain independent living for service users at Waverley Court, Forth Avenue, Portishead, BS20 7NY.

You will coordinate rosters, supervise Support Workers, and ensure high standards of care while promoting dignity, safety, and well-being for individuals.

Qualifications

• Minimum of one year in a Team Leader / Senior role.
• NVQ Level 2 in Health and Social Care required, with a willingness to work toward Level 3.
• Strong written and verbal communication skills.
• A proactive attitude, with the ability to adapt to the changing needs of the people we support.

Responsibilities

• Conducting supervisions, team meetings, and mentoring Support Workers to ensure the highest standard of care.
• Coordinating schedules and allocating tasks to maintain personalized support plans for each person.
• Supporting people with daily living activities such as medication, mobility, and personal care when needed.
• Building positive relationships within the extra care community.

About the Role

As a Team Leader in our Extra Care Service, you will play a pivotal role in delivering high-quality support to people living independently within their own homes in the extra care complex. Our approach emphasizes understanding and responding to the unique needs of each individual, helping them maintain their independence while ensuring their well‑being.

You will be the first point of contact for the care and support provided, with responsibilities including:
