Job summary

Radis Community Care is seeking an Extra Care Team Leader to join our team based at The Rise, George Lane, Plympton, Plymouth, Devon. You will lead care teams, oversee supervision and mentoring, and ensure personalised support plans for residents living in their own homes within the extra care complex.

You will coordinate schedules, support daily living activities, and build positive relationships within the community while upholding quality care standards and a person-centred approach.

Qualifications

• One year in a Team Leader/Senior role is required.
• NVQ Level 2 in Health and Social Care is required, with willingness to work toward Level 3.
• Strong written and verbal communication skills.
• Proactive attitude and adaptability to changing needs of people supported.

Responsibilities

• Conducts supervisions, team meetings and mentoring for Support Workers to maintain high care standards.
• Coordinates schedules and allocates tasks to maintain personalised support plans.
• Supports people with daily living activities such as medication, mobility and personal care when needed.
• Builds positive relationships within the extra care community.

About the Role

As a Team Leader in our Extra Care Service, you will play a pivotal role in delivering high-quality support to people living independently within their own homes in the extra care complex. Our approach emphasizes understanding and responding to the unique needs of each individual, helping them maintain their independence while ensuring their well-being.

You will be the first point of contact for the care and support provided, with responsibilities including:

• Conducting supervisions, team meetings, and mentoring Support Workers to ensure the highest standard of care.
• Coordinating schedules and allocating tasks to maintain personalized support plans for each person.
• Supporting people with daily living activities such as medication, mobility, and personal care when needed.
• Building positive relationships within the extra care community.
