Job summary

Radis Community Care is seeking an Extra Care Team Leader based at Diamond Batch, Weston Super Mare. You will lead a team delivering person-centred support in an extra care housing complex, helping residents maintain independence and well-being.

The role involves supervising staff, coordinating schedules, and supporting residents with daily living activities while building strong community relationships. A driving license is beneficial and Enhanced DBS is required.

Qualifications

• Minimum of one year in a Team Leader / Senior role.
• NVQ Level 2 in Health and Social Care with willingness to work toward Level 3.
• Strong written and verbal communication skills.
• A proactive attitude, with the ability to adapt to changing needs.

Responsibilities

• Conducting supervisions, team meetings, and mentoring Support Workers to maintain high care standards.
• Coordinating schedules and allocating tasks to maintain personalised support plans.
• Supporting people with daily living activities such as medication, mobility and personal care as needed.
• Building positive relationships within the extra care community.

About the Role

As a Team Leader in our Extra Care Service, you will play a pivotal role in delivering high-quality support to people living independently within their own homes in the extra care complex. Our approach emphasizes understanding and responding to the unique needs of each individual, helping them maintain their independence while ensuring their well-being.

You will be the first point of contact for the care and support provided, with responsibilities including:

• Conducting supervisions, team meetings, and mentoring Support Workers to ensure the highest standard of care.
• Coordinating schedules and allocating tasks to maintain personalized support plans for each person.
• Supporting people with daily living activities such as medication, mobility, and personal care when needed.
• Building positive relationships within the extra care community.
