Job summary

Radis Community Care is seeking an Extra Care Team Leader to join our team at The Rise, George Lane, Plympton, Plymouth, Devon, PL7 1LJ. You will lead a small team delivering person-centred care within an extra care housing complex, helping residents maintain independence and well-being.

You will supervise and mentor Support Workers, coordinate schedules, and support daily living activities such as medication and personal care.

Qualifications

• Minimum of one year in a Team Leader / Senior role.
• NVQ Level 2 in Health and Social Care required, with a willingness to work toward Level 3.
• Strong written and verbal communication skills.
• Proactive attitude and adaptability.

Responsibilities

• Conducting supervisions, team meetings, and mentoring Support Workers to ensure the highest standard of care.
• Coordinating schedules and allocating tasks to maintain personalized support plans for each person.
• Supporting people with daily living activities such as medication, mobility, and personal care when needed.
• Building positive relationships within the extra care community.

About the Role

As a Team Leader in our Extra Care Service, you will play a pivotal role in delivering high-quality support to people living independently within their own homes in the extra care complex. Our approach emphasizes understanding and responding to the unique needs of each individual, helping them maintain their independence while ensuring their well-being.

You will be the first point of contact for the care and support provided, with responsibilities include:

• Conducting supervisions, team meetings, and mentoring Support Workers to ensure the highest standard of care.
• Coordinating schedules and allocating tasks to maintain personalized support plans for each person.
• Supporting people with daily living activities such as medication, mobility, and personal care when needed.
