Job summary

Avery Healthcare Group in Leeds is seeking a warm, enthusiastic Team Leader to support the home management team in delivering high-quality care in a safe, caring and inclusive environment for residents, visitors and staff.

You will lead in person-centred care planning, support discharge processes, monitor residents' wellbeing and ensure care plans reflect changing needs while maintaining safe systems of work.

Qualifications

  • NVQ 3 or equivalent in care for care homes.
  • Two years' experience in a senior position with the resident group.
  • Ability to communicate effectively both verbally and in writing.

Responsibilities

  • Lead in the person-centred care planning process from development to audits and action plans.
  • Assist in discharge transitions, including documentation and updating next of kin.
  • Monitor residents' well-being and ensure changing needs are reflected in care plans.
  • Maintain safe systems of work and safe handling of residents, with risk assessment where needed.

We're now looking for a warm, enthusiastic, and dedicated individual to join our award-winning team as a Team Leader at Aire View Care Home in Leeds. If this sounds like the place for you, we'd love to hear from you!

About The Role

Your focus as Team Leader will be to support the home management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

Other Responsibilities Will Include
  • Leading in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
  • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
  • Leading with monitoring the well-being of each resident, being aware of any change impacting care and care planning and ensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.

Our Ideal Candidate Must
  • Have an NVQ 3 or equivalent in care for care homes.
  • Two years' experience working in a senior position with the resident group.
  • Have the ability to communicate effectively both verbally and in writing.
  • Demonstrate the ability to lead staff and act responsibly in the absence of the manager.
