Job summary

Avery Healthcare Group is hiring a Team Leader for Aire View Care Home in Leeds. You will support the home management team to deliver high-quality care in a safe, caring, and inclusive environment for residents, visitors, and staff.

Ideal candidates have NVQ 3 in care, at least two years in a senior role with residents, and strong written and spoken communication to lead a team effectively in the manager's absence.

Qualifications

  • NVQ 3 in care or equivalent is required.
  • Two years’ experience in a senior position with residents.
  • Strong communication skills, written and verbal.
  • Ability to lead staff and act responsibly in the absence of the manager.

Responsibilities

  • Lead person-centred care planning processes, monitor and audit care plans; develop action plans.
  • Assist in smooth discharge transitions, including documentation and informing relatives as appropriate.
  • Monitor residents' wellbeing, identify changes affecting care, and update records accordingly.
  • Practice safe working methods and safe handling of residents and staff, assessing risks.

ABOUT THE ROLE

Your focus asTeamLeaderwill be tosupportthehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open,supportive, andinclusiveenvironment for residents, visitors,and staff.


Other responsibilitieswillinclude:



  • Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.

  • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.

  • Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.

  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
