Avery Healthcare Group in the United Kingdom is seeking a dedicated Team Leader to support the home management team in delivering high-quality care within a safe, caring and inclusive environment for residents, visitors, and staff.

You will lead person-centered care planning, assist with discharge transitions, monitor resident wellbeing, and ensure safe working practices across the team and within the home. DBS checks are required and supported by Avery.

Qualifications

  • NVQ Level 3 (or equivalent) in care for care homes.
  • Two years' experience in a senior position with the resident group.
  • Ability to communicate effectively both verbally and in writing.
  • Demonstrate the ability to lead staff and act responsibly in the absence of the manager.

Responsibilities

  • Lead the home management team in delivering high-quality care and ensuring a safe, supportive environment.
  • Assist in the smooth discharge transitions including documentation and keeping residents' next of kin informed.
  • Monitor resident well-being and update care plans as needs change.
  • Ensure safe systems of work and safe moving and handling for residents and staff.

About the Role

Your focus as Team Leader will be to support the home management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

  • Leading in the person-centered care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
  • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
  • Leading with monitoring the well-being of each resident, being aware of any change impacting care and care planning and ensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
