Job summary

Avery Healthcare Group is seeking a warm, enthusiastic Team Leader for Acer Court Care Home in Nuthall, Nottinghamshire. You will support the home management team to deliver high-quality care in a safe, open and inclusive environment for residents, visitors and staff.

Applicants should have NVQ 3 in care, at least two years in a senior role with residents, and strong communication and leadership skills. ADBS disclosure checks and UK work eligibility are required.

We’re now looking for a warm, enthusiastic, and dedicated individual to join our award‑winning team as a Team Leader at Acer Court Care Home in Nuthall, Nottinghamshire.

ABOUT THE ROLE

Your focus as Team Leader will be to support the home management team in delivering high‑quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors and staff.

Other responsibilities will include:
  • Leading in the person‑centred care planning process by implementing, monitoring and undertaking monthly evaluations.
  • Completing care plan audits.
  • Developing action plans.
  • Being responsible for completing the action plans.
  • Assisting in the smooth transition of any discharges from the home, including documentation, medication and keeping the residents’ next of kin fully informed, where appropriate.
  • Leading with monitoring the well‑being of each resident, being aware of any change impacting care and care planning and ensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors and staff.

  • Have an NVQ 3 or equivalent in care for care homes.
  • Two years’ experience working in a senior position with the resident group.
  • Have the ability to communicate effectively both verbally and in writing.
