Job summary

Agincare in Grantham is seeking Support Workers to help deliver person-centered outcomes for individuals with complex needs, including learning disabilities, autism, and mental health conditions. The role is based in Grantham, Lincolnshire, supporting living and community independence.

You will work as part of a dedicated team to encourage participation, provide daily living support, and promote wellbeing with a person-centered approach.

Qualifications

• Experience supporting people with learning disabilities, autism, or mental health conditions.
• Ability to communicate using people’s preferred methods.
• Experience assisting with activities of daily living and promoting independence.

Responsibilities

• Encourage independence and community participation for people supported.
• Assist with daily living activities including personal care, meals, and routines.
• Communicate clearly using residents’ chosen methods.
• Engage and motivate individuals to improve quality of life.
• Support positive behavioural approaches where needed.
• Keep accurate records and report changes to management.
• Work as part of a team and use initiative as required.

You will need to be enthusiastic, have great people skills, and a positive and proactive approach to your role, supporting individuals with Learning Disabilities, Autism, Mental Health and complex needs. Where you'll be working: Grantham, Lincolnshire We are looking for Support Workers for our Services in Grantham. This is a unique service within Grantham offering supporting living, enabling people to live as independently as possible within their own homes and local community. We are looking for support workers to join our amazing team, and to continue delivering outstanding outcomes.

Key Responsibilities

• Encouraging people to play active roles in the communities in which they live, to encourage independence.
• Supporting Activities of daily Living including social activities, personal care, routines, planning and preparation of meals and budgeting.
• Communicating - effectively communicate using people's chosen methods.
• Engaging - promoting and motivating people we support to enhance their quality of life.
• Specialising - supporting people who may need positive behavioural support approaches.
• Recording - keeping accurate records & reporting changes in health or circumstances of service users to management.
• Team player - working as part of a team, as well as being able to work using your own initiative.
