Job Description

A career adventure with Achieve together provides the perfect opportunity to make a real difference. We are looking for team members who are passionate about delivering excellent care and support, celebrating achievements, and encouraging fun and creativity. Become a Support Worker with us and let’s shape lives. This is more than just a job – you’ll be helping the people we support gain independence and live healthy, happy and fulfilling lives.

Key Responsibilities

Enhancing and enriching quality of life for the people we support, enabling them to keep moving towards agreed goals and life skills in line with their individual activity planner.

Maintaining the general health, wellbeing and emotional needs of the people we support, including a high standard of personal care, attending health appointments and administering medication in line with company policies.

Complying with all health and safety requirements, safeguarding policies and best practice, and reporting any concerns around safety and safeguarding immediately.

Encouraging the people we support to access a range of opportunities and experiences, maximising their inclusion and participation in the community according to their needs, wishes and preferences.

Supporting the people we support to stay connected with family and friends and to participate in social and community activities, including annual holidays.

Maintaining accurate and detailed records in line with legislation and organisational policies, including care plans, risk assessments, daily reports and incident records.

Working with an up‑to‑date knowledge of each individual's preferred method of communication.

Working as part of a team, attending team meetings and contributing to a culture of open communication and constructive feedback.

Supporting behaviours that challenge safely and supportively, using approved approaches learnt through training.

Requirements

Although it would be beneficial to have some experience of healthcare work, it isn’t absolutely necessary as we provide full training. As long as you’re passionate, hard‑working and empathetic, you’ll fit right in.

As a Support Worker, you’ll ensure that the people we support are able to live their lives independently and engage in daily enrichment activities, while receiving focused, tailored support where needed.

This is not a role where you will be tied to a desk – you will enjoy the freedom to explore and access local communities and activities.

Care Certificate to be completed within the first 6 months in post.

Committed to completing 100% of allocated eLearning and service‑specific training.
