Job summary

Action Together is seeking a motivated Support Worker to join our homelessness accommodation service. The role supports individuals facing homelessness to address contributing factors and move toward independent living.

You will act as a key worker for a caseload, delivering person-centred support and coordinating interventions to promote positive outcomes. The role emphasizes detailed assessments, planning, and interagency collaboration.

Qualifications

  • GCSE English and Maths (or equivalent) required.
  • Experience with case management, planning and reviewing support.
  • Strong report-writing and administrative skills.
  • Ability to build professional relationships while maintaining boundaries.
  • Knowledge of housing and homelessness services or related field.
  • Experience with trauma-informed, person-centred approaches.

Responsibilities

  • Manage a caseload of residents and act as their designated key worker.
  • Complete comprehensive needs and risk assessments.
  • Develop and regularly review bespoke support plans.
  • Support residents to identify and achieve realistic goals.
  • Facilitate admissions, inductions, and move-ins.
  • Maintain accurate records, case notes and reports.
  • Liaise with external agencies including local authorities and housing providers.
  • Source move-on accommodation and support progression to independent living.
  • Monitor progress and ensure plans remain outcome-focused.
  • Work collaboratively as part of a team and promote safeguarding.

Job description

We are seeking a motivated and professional Support Worker to join our homelessness accommodation service. This is a rewarding but demanding role supporting individuals to address the issues that have contributed to their homelessness and to successfully move on to independent living.

The successful candidate will act as a key worker for a caseload of residents, providing person-centred support and coordinating interventions that promote positive outcomes.

Key Responsibilities
  • Manage a caseload of residents and act as their designated key worker.
  • Complete comprehensive needs and risk assessments.
  • Develop, implement, and regularly review bespoke support plans.
  • Support residents to identify and achieve realistic goals.
  • Facilitate admissions, inductions, and move-ins.
  • Maintain accurate, timely, and professional records, case notes, and reports.
  • Liaise with external agencies, including local authorities, housing providers, healthcare services, and employment support organisations.
  • Source suitable move-on accommodation and support residents through the transition to independent living.
  • Monitor progress and ensure support plans remain outcome-focused.
  • Contribute to the effective operation of the service and work collaboratively as part of a team.
  • Promote safeguarding, equality, diversity, and professional boundaries at all times.

  • Excellent written and spoken English.
  • Strong report-writing, record-keeping, and administrative skills.
  • A good standard of education (minimum GCSE or above in English and Mathematics, or equivalent).
  • Experience of case management, key working, support planning, housing support, homelessness services or a related field.
  • Ability to build professional relationships while maintaining clear boundaries.
  • Strong organisational skills and attention to detail.
  • Competence in using Microsoft Office and case management systems.
  • Ability to work independently and manage competing priorities.
  • Experience working with people experiencing homelessness.
  • Knowledge of housing pathways, welfare benefits, and tenancy sustainment.
  • Understanding of trauma-informed and person-centred approaches.

This is not a personal care role. The position focuses on casework, support planning, housing outcomes, and helping residents progress towards independent living. Candidates must be confident completing detailed written assessments, maintaining accurate records, and communicating professionally with residents and partner agencies.
