Job summary

Thirteen Group is seeking a Support Service Co-ordinator for a 9-month secondment, with a possible 3-month extension. The post is based at School View, Durham, supporting young families to live more independently through person-centred plans and weekly keywork sessions.

You will help manage risk, safeguarding, referrals, and provide reports. The role covers two-week rotas, occasional Saturdays, and on-call duties, within a collaborative four-person team.

Qualifications

• Experience in supporting vulnerable client groups and safeguarding responsibilities.
• Ability to work lone and as part of a team in a care setting.
• Strong interpersonal skills and resilience when dealing with challenging situations.

Responsibilities

• Run activities that reduce social isolation and support learning for customers.
• Assess referrals and determine suitability for the service.
• Create person-centered support plans and conduct weekly keywork sessions.
