Job summary

Thirteen Group is seeking a Support Service Co-ordinator on a 9-month secondment based at School View in Durham to support young families and help them live more independently. You will join a small team, manage referrals, develop person-centred support plans, monitor safeguarding, and attend multi‑agency meetings as part of a responsive housing and welfare service.

Qualifications

• Experience of supporting vulnerable client groups, with safeguarding understanding.
• Ability to work lone and in a team.
• Driving licence and access to own vehicle for outreach work across County Durham.

Responsibilities

• Run activities to improve social isolation and learning linked to support, with creative freedom to deliver positive outcomes.
• Support referrals and assess suitability for the service.
• Create person-centred support plans and conduct weekly keywork sessions to achieve goals.
• Manage risk and safeguarding for residents; attend multi-agency meetings and provide written reports.
• Manage void properties and allocations; ensure building compliance and health & safety.
