Job summary

Home Group in Huddersfield is seeking a dedicated care professional to help people with complex needs live more independently. You will lead planning, coordinate support workers, and balance safeguarding with administrative duties while guiding colleagues.

Qualifications

• Passion and experience in supporting others, ideally in a supported housing setting.
• Level 3 Diploma in Care or equivalent experience (or willing to work towards it).
• Experience assessing referrals and coordinating support.
• Ability to work under your own initiative, stay calm under pressure, and be resilient.

Responsibilities

• Lead support planning and coordinate our Support Workers to deliver person-centred support.
• Balance safeguarding, admin and meetings while guiding colleagues and managing caseloads.
• Support customers with daily tasks like budgeting, cooking and scheduling appointments.
• Work with multi-agency teams to safeguard and empower customers.
• Help customers build independence and prepare for move-on into the community.
