Job summary

Home Group Limited is hiring a Support Coordinator in Middlesbrough. The role focuses on leading support planning, coordinating Support Workers to deliver person-centred assistance, and safeguarding customers within a housing support setting.

You’ll assess referrals, coordinate services, and help clients with daily tasks while building independence and planning move-on, in a full-time on-site position with a strong focus on person-centred care.

Qualifications

• Experience in supported housing setting.
• Level 3 Diploma in Care or equivalent experience.
• Experience assessing referrals and coordinating support.

Responsibilities

• Lead support planning and coordinate our Support Workers to deliver person-centred support.
• Balance support, safeguarding, admin and meetings while guiding colleagues and managing caseloads.
• Support Customers with daily tasks like budgeting, cooking and attending appointments.
• Work with multi-agency teams to safeguard and empower our customers.
• Help customers build independence and prepare for their move-on into the community.
