Job summary

Home Group in Sunderland is seeking a dedicated support worker to help people with complex mental health needs live more independently in their own homes. You’ll be part of a caring team that puts customers first and values wellbeing as part of everyday practice.

You’ll lead support planning, coordinate colleagues, and work with multi-agency teams to safeguard and empower customers, supporting budgeting, appointments and daily living. A flexible approach and resilience are essential.

Qualifications

• Level 3 Diploma in Care (or equivalent) required.
• Experience assessing referrals and coordinating support.
• Passion for helping others.
• Ability to work under pressure with resilience.

Responsibilities

• Lead and coordinate Support Workers to deliver person-centred support.
• Balance safeguarding, admin, and meetings while managing caseloads.
• Support customers with budgeting, daily tasks and appointments.
• Collaborate with multi-agency teams to safeguard customers.
• Help customers move toward independence in the community.
