Job summary

Home Group in the United Kingdom is seeking a dedicated role to lead support planning and coordinate our Support Workers to deliver person-centred care. You will balance safeguarding, admin and meetings while guiding colleagues and managing caseloads.

You will support customers with daily tasks such as budgeting, cooking, and appointments, and work with multi-agency teams to safeguard and empower people. A hybrid role includes service and home-based work.

Qualifications

• Passion and experience in supporting others ideally in a supported housing setting.
• Level 3 Diploma in Care or equivalent (or willing to work towards it).
• Experience assessing referrals and coordinating support.

Responsibilities

• Lead support planning and coordinate our Support Workers to deliver person-centred support.
• Balance support, safeguarding, admin and meetings while guiding colleagues and managing caseloads.
• Support Customers with daily tasks like budgeting, cooking and attending appointments.
• Work with multi-agency teams to safeguard and empower our customers.
• Help customers build independence and prepare for their move-on into the community.
• Maintain a 5 week rolling rota and cover early mornings, days, evenings, and nights as required.
• Hybrid role with service and home-based work; attend training and team meetings.
