Job summary

Home Group Limited in Huddersfield seeks a Support Coordinator to lead planning and coordinate Support Workers, delivering person-centred care to customers with complex needs. The role is permanent and full-time, offering 34 days leave (rising to 39) and a health cash plan worth over £1,140.

You will support independence, manage caseloads, collaborate with multi-agency teams and help customers move toward community living. Salary £27,500–£28,300 per year.

Qualifications

• Experience supporting others, ideally in supported housing or care settings.
• Ability to assess referrals and coordinate support.
• Proactive, calm under pressure with initiative.

Responsibilities

• Lead support planning and coordinate Support Workers to deliver person-centred care.
• Balance support, safeguarding, admin and meetings while guiding colleagues and managing caseloads.
• Support customers with daily tasks such as budgeting, cooking and appointments.
• Work with multi-agency teams to safeguard and empower customers.
• Help customers build independence and prepare for move-on into the community.
