Typical day as a Support Coordinator
  1. Working under the support and supervision of your Senior Client Service Manager and Senior Support Coordinator, to ensure our customers receive the necessary support to develop their independence and enable them to reach their desired outcomes.
  2. Supporting customers with varied support needs including mental health issues, housing related support, complex needs and responding appropriately to challenging behaviour.
  3. You will create and update support plans and risk assessments with your customers. Helping them with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation.
  4. Reviewing support plans and risk assessments when required to ensure your customers needs are being met.
  5. Ensuring our properties are well maintained, clean and homely. This not only ensures our customers have a nice place to live, but it also helps with keeping our voids to a minimum.
  6. Cuppas, chats and catch ups. There’s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You have
  • Experience of creating person-centred support plans, risk assessments and reviews
  • Experience of coordinating and assessing customer referrals.
  • The ability to work on your own initiative, remain calm under pressure and have a resilient approach.
  • The understanding of where our customers are in their life and have the passion to advocate for them.
  • Experience of delivering support in a housing or care environment
  • To get from A to B, you’ll need a vehicle insured for business purposes. The great news is that we’ll pay your mileage!

Job details
  1. The service is staffed 24 hours a day, 7 days a week. You will work on a 4 week, rolling rota, including weekends, and bank holidays. This rota does not require you to work night shifts.
  2. Occasionally, there may be a requirement for you to work flexible hours in order to meet the ever-changing needs of our customers.
  3. Morning and late shifts may include a period of time where you will be lone working.
  4. Basic knowledge/aptitude of Microsoft Word and Excel is essential.
  5. Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
  6. You’ll need an Enhanced DBS check done and we pay for that.
