Job Description - Mental Health Support Coordinator (25480)

Location: North Tyneside Transitions- North Shields

Type: Permanent, full time (37.5 hpw), working on a 2 week rolling rota.

Pay: £13.21 per hour (salary £25,838 per annum) and great benefits including Health Cash Plan

As a Mental Health Support Coordinator, you will be part of a team of support coordinators, supporting our customers who have complex mental health needs to live independently. You’ll make a big difference each day as you help our customers achieve their hopes and aspirations.

Typical day as a Mental Health Support Coordinator

  • Supporting customers with daily living activities such as shopping, budgeting, and maintaining their tenancy.
  • Carrying out risk assessments, support planning, goal setting, and regular reviews.
  • Engaging in conversations and catch-ups with customers.
  • Liaising with various professionals and attending Multi-disciplinary team meetings.
  • Recording in customers' daily notes.

  • The ability to work on your own initiative, remain calm under pressure, and have a resilient approach.

Due to limits on our current certificates of sponsorship, we are not able to offer sponsorship to external candidates for this role. This remains under regular review.

Job details

  • Flexibility is key; you may need to adapt your start or finishing times to meet customers' needs.
  • You’ll work on a two-week rolling rota, which is 4 shifts one week and 3 shifts the next.
  • Regular training and team meetings are part of the role.
  • You’ll need the ability to use technology for creating and updating support plans and completing online learning.
  • An Enhanced DBS check is required, and we will cover the cost.
