Epsom and St Helier University Hospitals NHS Trust is seeking a Patient Pathway Coordinator - Stroke to join its administrative team in Epsom. The post supports coordination and scheduling of outpatient activity for the Stroke Service and provides inpatient support as required.

Reporting to the Service Manager for Stroke, you will handle patient pathways from referral to treatment and/or discharge, manage queries from patients and clinicians, and maintain high standards of customer service.

• Coordinate outpatient activity for the Stroke Service and support inpatient activity.
• Track and support patients from referral to treatment and/or discharge.
• Respond to queries from patients, GPs and clinicians; provide day-to-day administrative support.
• Ensure high standards of customer service and effective telephone handling.
