Social Worker – Hospital Team

Salary: £38,976 - £52,767
Location: Wandsworth, London
Contract: Permanent, Full Time (36 hours)
Service: Adult Social Care - Hospital Team (Wolfson Neuro-Rehabilitation Unit)

About the Role

A rare opportunity has become available for a Social Worker to join the Wandsworth Hospital Team, based within the Wolfson Neuro-Rehabilitation Unit at Queen Mary's Hospital. This is a specialist setting supporting adults with complex physical, cognitive, and communication needs. In this role, you will deliver a high-quality hospital discharge service that promotes independence, well-being, and positive outcomes for residents and their carers.

Key Responsibilities
  • Complete holistic assessments, manage risks, and make proportionate, evidence-based decisions.
  • Support safe and timely hospital discharges, including complex arrangements and housing applications.
  • Manage referrals, maintain patient flow, and act as a safeguarding enquiry officer.
  • Work closely with a multi-disciplinary team, attending weekly MDT meetings and Goal Planning sessions.
  • Liaise effectively with external agencies and local authorities, including supporting patients from other boroughs.
  • Contribute to a person-centred, strengths-based approach that empowers residents and families.
About You
  • Qualified, SWE-registered Social Worker.
  • Strong post-qualifying experience within adults' social care.
  • Confidence working with individuals with physical, cognitive, and communication impairments.
  • Excellent communication, decision-making, and time management skills.
  • Proactive, solution-focused approach and commitment to high-quality practice.
