What you will do
  • Deliver high-quality, person-centred social work practice in accordance with the Care Act 2014 and associated legislation.
  • Manage a caseload of adults with learning disabilities and autism, ensuring the effective delivery of care.
  • Undertake statutory duties, including assessments, support planning, reviews, and risk assessments.
  • Participate in team duty systems, responding to new referrals, emergencies, and safeguarding concerns.
  • Conduct safeguarding enquiries following local multi-agency procedures, emphasizing Making Safeguarding Personal.
  • Identify and assess risk, completing risk assessments in line with Merton Care Management Procedures.
  • Foster effective collaborations with internal and external partners to ensure robust care planning and service delivery.
