Become part of the Hospital Team, where you'll play a vital role in supporting residents through a safe and well‑coordinated discharge process. Your efforts will help ensure that individuals transition smoothly from hospital care back into the community, with the right support in place to maintain their wellbeing and independence.

Join the Community Team and take on a rewarding role where you’ll handle more complex cases and contribute to case reviews. Your work will help ensure that residents are supported in living their lives to the fullest. This is a full‑time, permanent role with hybrid working, and a salary of £38,192 - £42,012 per annum, plus an annual retention bonus of £3,000 subject to eligibility.

This is an excellent opportunity for an experienced Social Worker to join our Hospital and Community teams.

The Role

As a Social Worker within these teams you will be helping to support adults to live independently and safely by assessing needs, coordinating care, and safeguarding wellbeing. Work closely with individuals, families, and partner agencies to promote dignity, choice, and empowerment.

You will be supported with your caseload by a Senior Social Worker and a Team Manager.

Your role will involve:
  • Support & Advocacy: They help adults who may be vulnerable due to age, disability, illness, or mental health challenges to live independently and with dignity.
  • Assessment & Planning: They assess individuals’ needs and develop care plans tailored to improve wellbeing and safety.
  • Safeguarding: They protect adults from abuse, neglect, or exploitation, working closely with other agencies when risks are identified.
  • Coordination: They liaise with families, carers, health professionals, and community services to ensure holistic support.
  • Empowerment: They promote choice and control, helping people understand their rights and access services.
What we are looking for
  • Relevant Qualification & Registration: Degree in Social Work and registration with Social Work England.
  • Assessment & Care Planning Skills: Ability to carry out strengths‑based assessments and develop person‑centred care plans.
  • Safeguarding Knowledge: Strong understanding of adult safeguarding procedures and legislation (e.g., Care Act 2014).
  • Communication & Collaboration: Excellent interpersonal skills to work effectively with individuals, families, and multi‑agency teams.
  • Resilience & Professionalism: Capacity to manage complex cases, maintain professional boundaries, and adapt to changing demands.
