Adullam Homes Housing Association is seeking a Service Coordinator to join the national support team. The role focuses on adults with complex needs, delivering person-centred support to help secure independent living and housing options.

Shifts operate on a rota for a 24-hour service, averaging 37.5 hours per week, with 9am–10pm weekday coverage and some weekend work. DBS enhanced check and UK right-to-work are required.

Formación

• Experience recording and reporting case notes on an online CRM system.
• Awareness of issues affecting vulnerable people and those facing multiple disadvantage.
• Flexible, resilient and therapeutic approach to working with people.
• Practical skills supporting service users in daily living for independence.
• Ability to motivate and empower service users.

Responsabilidades

• Support people with multiple and complex needs who have lost their home or are rough sleeping.
• Build a tailored, person-centred support package for each service user.
• Help service users develop the skills to move on to independent living or permanent housing.
• Manage a caseload of service users.
• Contribute to the rota staffing a 24-hour service.
• Support service users with activities of daily living needed for independence.
• Motivate and empower service users to have greater control and choice over their lives.
• Record and report case notes on an online CRM system.
• Identify, monitor and report safeguarding concerns.
• Deliver activities and support sessions, including some weekend work.
• Work within the organisation's values and ethos.
