Job summary

Adullam Homes Housing Association is seeking a Service Coordinator to support people with multiple and complex needs who are homeless or at risk. You will develop person-centred support plans to help service users move into independent living and permanent housing, tailored to individual goals.

The role operates on a 24‑hour rota, 37.5 hours per week with shifts 9am–10pm on weekdays and some weekend activities.

Qualifications

• Experience managing a caseload of service users.
• Awareness of issues affecting vulnerable individuals with multiple disadvantages.
• Ability to help service users identify goals and achieve outcomes.
• Experience recording and reporting case notes in an online CRM.

Responsibilities

• Manage a caseload of service users and provide tailored support.
• Record and maintain case notes using the CRM system.
• Support service users in activities of daily living to promote independence.
• Adhere to safeguarding policies and report safeguarding concerns promptly.
