Job summary

NELFT NHS is seeking a Senior Rehabilitation Assistant to join the inpatient team. You will support multidisciplinary colleagues to deliver rehabilitation programmes for adults with frailty and complex health needs.

You will assist with mobility, transfers and daily living activities, promote independence, monitor progress and help with discharge planning and equipment coordination on the ward.

Qualifications

• Deliver delegated rehab interventions under the direction of Physiotherapists and Occupational Therapists.
• Support mobility, transfers, activities of daily living and therapeutic exercise.
• Promote independence, recovery and confidence to maximise function.
• Monitor and report patient progress and changes to staff.
• Support discharge planning and arrange rehabilitation equipment.
• Act as contact for patients, relatives and carers on goals and plans.
• Coordinate therapy timetables and ward rehabilitation activities.
• Support junior Rehabilitation Assistants and students.
• Contribute to service development, audits and quality improvement.
• Work with multidisciplinary team to ensure safe, patient-centered care.

Responsibilities

• Supporting patients with mobility, transfers, activities of daily living and therapeutic exercise.
• Promoting independence, recovery and confidence to maximise patients' functional potential.
• Monitoring and reporting patient progress and any changes in presentation to registered staff.
• Supporting discharge planning, including arranging and coordinating rehabilitation equipment.
• Acting as a point of contact for patients, relatives and carers, responding to concerns and supporting communication regarding rehabilitation goals and discharge plans.
