Job summary

NELFT North East London Foundation Trust is seeking a Senior Rehabilitation Assistant (Band 4) for Mountnessing Court, Billericay. You will join a multidisciplinary therapy team delivering high-quality, person-centred rehabilitation to adults with frailty and complex health needs.

You will work under Physiotherapists and Occupational Therapists, manage your workload, and contribute to discharge planning and coordinated rehabilitation activities across sites.

Responsibilities

• Delivering delegated rehabilitation interventions under the direction of Physiotherapists and Occupational Therapists.
• Supporting patients with mobility, transfers, activities of daily living and therapeutic exercise programmes.
• Promoting independence, recovery and confidence to maximise patients' functional potential.
• Monitoring and reporting patient progress and any changes in presentation to registered staff.
• Supporting discharge planning, including arranging and coordinating rehabilitation equipment where appropriate.
• Acting as a point of contact for patients, relatives and carers, responding to concerns and supporting communication regarding rehabilitation goals and discharge plans.
• Coordinating and supporting therapy timetables and rehabilitation activities on the ward.
• Supporting junior Rehabilitation Assistants, students and new members of staff.
• Participating in service development, audit and quality improvement initiatives.
• Working collaboratively with the multidisciplinary team to ensure safe, effective and patient-centred care.
