Job summary

NELFT is seeking a Community Rehabilitation Assistant to support adults in their homes within the Waltham Forest area. • Support implementation of individualised care and treatment plans under guidance of qualified staff.

• Promote independence, dignity and choice in all interactions.
• Monitor and report changes in patient condition or progress.
• Liaise with internal teams and external agencies to support safe, effective care delivery.
