Job summary

Change Grow Live in the UK is seeking a Senior Worker/Practitioner to assess and engage service users with complex needs, ensuring the right intervention at the right time. You will work within a multi-disciplinary team and act as a practice lead within the service to support recovery and social reintegration.

Based in Wirral, the role is 37.5 hours per week, on a fixed-term contract of 12 months. You will manage a challenging caseload, adhere to information governance and DBS requirements, and

Qualifications

• Experience in drug and alcohol issues.
• Experience in substance misuse services.
• Excellent verbal and written communication.
• Strong time management and prioritisation.
• Understanding of mental health interventions.
• Collaborative teamwork with diverse professionals.
• Adherence to information governance and safeguarding.

Responsibilities

• Provide unwavering support to service users from initial contact through recovery.
• Hold a caseload of complex service users and deliver high-quality care.
• Screening, assessment, and recovery planning with onward referrals.
• Implement tailored interventions to reduce harm from opiate use.
• Ensure a comprehensive recovery-focused plan including post-care support.
• Collaborate with partner organisations and mutual aid networks.
• Advocate for involvement of families and communities.
• Serve as a voice for service users in accessing services.
• Promote social reintegration and recovery-focused outcomes.
