Job summary

Change Grow Live in the UK is seeking a Recovery Coordinator to facilitate seamless transitions for service users referred from hospitals. You’ll work across sites including Kettering General Hospital, applying harm-reduction and psychosocial interventions to support treatment pathways.

We welcome applicants who understand substance misuse, are strong communicators, and can manage time effectively; full-time 37.5 hours, permanent contract with pro rata salary range £28,558–£32,802.

Qualifications

• Strong understanding of substance misuse and related interventions.
• Excellent verbal and written communication with good IT skills.
• Proactive, able to work independently with good time management.

Responsibilities

• Work in hospital settings to engage and support service users with drug/alcohol needs.
• Provide follow-up treatment and referrals across healthcare sites.
• Collaborate with partner agencies to meet unmet needs.
• Screen, assess and plan recovery with onward referrals.
