Qualifications

• Strong understanding of Care Act and Mental Capacity Act.
• Ability to support and develop staff within a health care setting.

Responsibilities

• Lead complex hospital discharge cases and high-risk decision-making.
• Complete and quality-assure Care Act and MCA assessments.
• Provide professional guidance, oversight and supervision to staff.
• Lead safeguarding activity and multi-agency working.
• Support timely, lawful and person-centred hospital discharges.
• Work closely with NHS and partner agencies to reduce delays.
• Ensure compliance with legislation, policy and best practice.
