Job summary

The Primary Care Alliance CIC is recruiting a GP Clinical Lead for the Berkshire West area to provide senior clinical leadership for the PCS service. The role focuses on frailty, deprescribing, and coordinated access to broader health and social care across Reading and surrounding areas.

The post requires Full GMC registration and NHS Performers List inclusion, with involvement in governance, SOPs, and caseload management. Fixed-term, part-time, 6 months, with flexible working patterns.

Qualifications

• Essential knowledge of frailty, multimorbidity, polypharmacy and deprescribing.
• Strong senior clinical decision-making and balancing safety with patient independence.
• Ability to develop SOPs and escalation pathways.
• Ability to collaborate across primary care, acute and community services.

Responsibilities

• Provide senior clinical leadership for PCS and act as principal clinical lead for the service.
• Support safe mobilisation and implementation with the Chief Delivery Officer and operational team.
• Lead development and maintenance of clinical SOPs, caseload processes and escalation pathways.
• Oversee GP and prescribing clinician workforce and ensure consistent clinical decision making.
• Support complex case discussions and escalation when needed.
• Ensure geriatric input is accessible for complex frailty and multimorbidity.
• Oversee assessment, falls, medication optimisation, ReSPECT, CGA and disease optimisation.
• Define caseload management model and safe step-down when stable.
• Shape interfaces with UCR, SDEC, acute geriatrics and VCSE partners.
• Contribute to evaluation and refinement of the model at 3, 6, 9 and 12 months.
• Promote test-and-learn culture and data-driven improvements.
