Job summary

Dorking Healthcare Ltd. is seeking a Frailty General Practitioner to manage the PCN frailty pathway, offer in-home care, and lead a multidisciplinary team. The role involves triage, care planning, and collaboration with community and hospital services.

The post supports clinical leadership within the frailty hub and wider community team, with regular supervision and opportunities for development. Travel between sites is required.

Qualifications

• UK-registered GP with experience in primary care and frailty management.
• Experience leading MDT meetings in Primary Care and NHS settings.
• Willingness to travel to multiple sites and meet patients at home or care homes.
• Knowledge of GP clinical systems EMIS and/or TPP SystmOne.

Responsibilities

• Proactively triage and manage frail patients referred by the PCN.
• Develop care plans to reduce hospital admissions and improve patient care.
• Provide clinical leadership to the frailty hub and medical leadership to the community team.
• Collaborate with Community Matrons, District Nurses, Pharmacists and other providers.
• Maintain accurate records and participate in governance and training.
