Job summary

Bridgwater Bay PCN is seeking enthusiastic, patient-centred GPs to join our Proactive Care Team. The role focuses on caring for and supporting patients living with frailty and long-term conditions in their own homes, with support from an experienced GP and other AHPs with frailty expertise.

Qualifications

• Hold full GMC registration with a licence to practise.
• Included on NHS England GP Performers List.
• Experience within Somerset is desirable.
• Adaptable and keen to experience different practice environments.
• Value high-quality patient care and teamwork.

Responsibilities

• Deliver proactive care to patients in the community.
• Review frailty patients, including housebound and those with long-term conditions.
• Deliver high-quality consultations in patients' homes.
• Build relationships with team members, local practices and patients.
