Job summary

Launceston Medical Centre is seeking a Principal GP to lead the Frailty Test and Learn project within the Launceston and Tamar Valley Integrated Neighbourhood Team. You will combine clinical frailty work with service development and clinical leadership, contributing to early identification and coordinated care for older adults.

The role is fixed-term for 18 months with part-time, flexible hours and a strong emphasis on multidisciplinary teamwork, safeguarding and governance.

Qualifications

• Qualified GP with GMC registration and UK eligibility.
• Experience in frailty, geriatrics or complex care.
• Ability to lead and contribute to service development and MDT working.

Responsibilities

• Review patient records to identify proactive frailty interventions.
• Conduct proactive reviews by face-to-face or telephone, including medication review and falls assessment.
• Provide medical review for complex frailty cases and ensure continuity of care.
• Support MDT with clinical governance, escalation and decision-making.
• Undertake home visits when clinically necessary in collaboration with the team.
