Job summary

Somerset LMC’s Bridgwater Bay Primary Care Network is seeking enthusiastic GPs on a fixed-term contract (6–12 months initially) to join the Proactive Care Team. The role focuses on caring for and supporting patients living with frailty and long-term conditions in their own homes, with backing from an experienced GP and other AHPs with frailty expertise.

There is flexibility to work across local GP practices during the winter period to provide additional sessions and capacity.

Location: Bridgwater Bay Primary Care Network
Contract: Fixed Term (6-12 months initially)
Hours: Flexible – we welcome a range of working patterns and commitment, from one to six sessions

We are looking for enthusiastic and patient-centred GPs to join our Primary Care Network (PCN) as soon as they are able, to work within our Proactive Care Team. The role will be focussed on helping to care for and support patients living with frailty and long term conditions in their own homes. There is support from an experience GP with an interest in frailty, and other AHPs with an interest and experience in frailty.

Additionally, we have flexibility for GPs to work within local GP practices over the winter period should they desire, providing additional sessions and capacity.
