Qualifications

• Current UK medical registration is required.
• Experience in frailty and complex care is desirable.
• Experience leading multidisciplinary teams is essential.

Responsibilities

• Provide senior GP clinical input and leadership within the Integrated Neighbourhood Team.
• Support care for patients living with frailty and complex needs across member practices.
• Lead multidisciplinary meetings and care planning.
• Develop and deliver proactive frailty pathways and integrated neighbourhood care.
• Ensure patient-centred care and reduce hospital admissions where possible.
