Job summary
The Meridian PCN is seeking an experienced Frailty Lead to support home visits and ongoing care across our PCN practices in Cambridgeshire and surrounding areas. The role includes mentoring Nursing Associates and HCAs, with autonomy to shape community-based care and respond to evolving ICB and neighbourhood frailty initiatives.
Ideally, the role is filled by a prescribing Advanced Nurse Practitioner or Advanced Care Practitioner, working across surgery and community settings with a strong focus
Qualifications
- Manage own caseload for older patients in surgery and community.
- Engage individuals as active partners in their treatment and support.
- Provide mentorship and clinical support to Nursing Associates and HCAs.
- Work with a high degree of autonomy in community settings.
Responsibilities
- Work with older people in surgery and out in the community including care homes.
- Manage own caseload and engage patients in their treatment and support.
- Support onward referral to secondary and specialist services where indicated.
- Develop close relationships with surgery teams and maintain records.
- Keep accurate written and electronic records using SystmOne and EMIS.
- Promote safeguarding of vulnerable adults.
