Leaders in Care is seeking a Complex Care Case Manager to oversee community-based care packages in Merseyside. You will manage your caseload, assess needs, plan and mobilise care, and lead care teams with a focus on high-quality, person-centred support in clients’ homes.

The role emphasizes building strong relationships with clients and families, ensuring CQC compliance and ongoing service improvement, with a Monday–Friday pattern and a salary up to £35,000 plus bonus and on-call payments.

Formación

• Experience coordinating or managing complex care packages.
• Knowledge of neurological conditions, physical disabilities, or long-term complex healthcare needs.
• Experience supervising and supporting care teams.
• Good understanding of CQC regulations and safeguarding.

Responsabilidades

• Manage a caseload of community-based complex care packages.
• Complete care assessments, care plans and risk assessments.
• Coordinate and mobilise new care packages.
• Lead, supervise and develop community care teams.
• Build strong relationships with clients, families and healthcare professionals.
• Ensure compliance with CQC standards and safeguarding requirements.
• Monitor care quality and drive continuous improvement.
