Job summary

Leaders In Care Ltd is recruiting a Nurse Case Manager to oversee adult complex care packages across Merseyside and West Lancashire, enabling people to live safely at home. The role offers autonomy, Monday–Friday hours and hybrid working with office and client/home visits.

You will manage assessments, care planning, and ongoing reviews while leading care teams and ensuring high standards of clinical governance. Driving licence is required.

Qualifications

• Holistic clinical assessment and care planning
• Experience with complex care packages in the community
• Mentoring or supervising care teams
• Autonomy in community settings
• Tracheostomy and ventilator care experience
• Full UK driving licence required

Responsibilities

• Own a caseload of complex care packages from initial assessment to clinical review
• Develop personalised care plans for adults in the community
• Review and manage complex care packages in collaboration with families and MDTs
• Provide clinical leadership and supervision to care teams
• Ensure governance, quality and compliance across care delivery
• Identify improvements to care delivery and outcomes
