Job summary

Leaders In Care Ltd is seeking an Adult Nurse Case Manager for a Fazakerley-based role supporting Merseyside and West Lancashire. You’ll manage complex care packages, lead clinical governance and visit clients at home. The post requires an active NMC registration and strong assessment skills, with a Monday-Friday pattern and no nights.

Qualifications

• Registered General Nurse with active NMC registration.
• Experience in complex care, including tracheostomy and ventilator care.
• Proven ability to supervise or mentor care staff.
• Strong clinical assessment and care planning skills.
• Must hold a full UK driving licence.

Responsibilities

• Manage and review complex care packages for adults.
• Complete clinical assessments and develop care plans.
• Visit clients in their own homes to monitor care.
• Provide clinical leadership to care teams.
• Support and mentor nurses and support workers.
• Collaborate with families and other healthcare professionals.
• Ensure high standards of clinical governance and care.
