Job summary

Leaders in Care is seeking an experienced Adult Nurse Case Manager to oversee complex care packages across Bristol and North Somerset, enabling a shift from hospital to home-based care while applying critical care expertise.

You will manage caseloads, provide clinical leadership to care teams, and support families to ensure high-quality, personalised in-home care for adults with complex needs. Monday–Friday, with potential home working.

Qualifications

• Registered NMC Adult Nurse (RGN) in the UK
• Experience in Intensive Care (ITU/ICU) or community complex care
• Strong knowledge of tracheostomy care and ventilation
• Excellent communication, clinical assessment and organisational skills
• Full UK driving licence and willingness to travel across Bristol and North Somerset

Responsibilities

• Managing a caseload of adult complex care packages across Bristol and North Somerset
• Completing initial clinical assessments and ongoing patient reviews
• Overseeing care for patients requiring tracheostomy care, ventilation (invasive and non-invasive)
• Developing and reviewing individualised care plans and clinical risk assessments
• Providing clinical supervision and support to care teams
• Delivering clinical training and assessing staff competencies
• Conducting clinical audits and maintaining high standards of clinical governance
• Working with patients, families and multidisciplinary professionals
• Supporting recruitment, induction and development of care staff
