Job summary

Leaders in Care in Merseyside seeks a Complex Care Case Manager to oversee community-based care packages across Merseyside on a permanent, full-time basis. You will manage and develop care teams, ensuring high-quality, person-centered support in clients’ homes.

You will assess needs, coordinate packages, monitor quality, and maintain excellent relationships with clients and families while ensuring CQC compliance. This role offers a clear path for progression.

We're recruiting for a Complex Care Case Manager to join an established specialist care provider, overseeing community-based care packages across Merseyside.

Your Role
