Job summary

Harmony Healthcare is seeking a Clinical Case Manager to lead and coordinate high-quality, person-centred care for clients in their own homes. You will oversee clinical care, work with commissioners to develop new packages, and support staff to ensure continuity and safety in all services.

The role requires an NMC-registered nurse with at least five years’ experience in health and social care, including community-based complex care.

Qualifications

• Registered Nurse with a valid NMC PIN.
• Minimum of 5 years' experience in health and social care, including community-based complex care.
• Strong leadership, team management and problem-solving skills.
• Excellent communication, organisational and IT skills.

Responsibilities

• Assess the physical and mental health needs of clients and create tailored care plans.
• Complete risk assessments and ensure a safe care environment.
• Act as a key contact for external agencies, office teams, clients and families.
• Conduct reviews to adapt care packages to evolving client needs.
• Provide training, supervision and appraisals for healthcare staff.
• Support recruitment, interviewing and onboarding of healthcare professionals.
