Job summary

Leaders in Care is recruiting a Complex Care Case Manager for West Lancashire. You will oversee high-quality home-based care packages, coordinate assessments, plans and risk reviews, and lead care teams to deliver tailored support.

You’ll build strong client and family relationships while ensuring safeguarding and CQC compliance. The role is Monday–Friday, full-time, with a salary up to £35,000 plus bonus and on-call payments, plus 25 days leave and pension.

Qualifications

• Experience coordinating or managing complex care packages.
• Knowledge of neurological conditions, physical disabilities or long-term complex healthcare needs.
• Experience supervising and supporting care teams.
• Good understanding of CQC regulations and safeguarding.
• Strong organisational, communication and leadership skills.
• Full UK driving licence and access to your own vehicle.

Responsibilities

• Managing a caseload of community-based complex care packages.
• Completing care assessments, care plans and risk assessments.
• Coordinating and mobilising new care packages.
• Leading, supervising and developing community care teams.
• Building strong relationships with clients, families and healthcare professionals.
• Ensuring compliance with CQC standards and safeguarding requirements.
• Monitoring care quality and driving continuous improvement.
