Job summary

Leaders in Care is recruiting a Complex Care Case Manager to oversee high-quality care packages across West Lancashire. You will manage a caseload, coordinate assessments and care plans, and work with clients, families and health professionals to ensure personalised support.

The role emphasises governance under CQC standards, safeguarding, leadership of community care teams, and driving continuous service improvement.

Qualifications

• Experience coordinating complex care packages.
• CQC regulations and safeguarding knowledge.
• Full UK driving licence and access to own vehicle.
• Strong organisational, communication and leadership skills.

Your Role

As a Complex Care Case Manager, you'll oversee the delivery of high-quality care packages for individuals with complex healthcare needs across West Lancashire.

Responsibilities

About You

• Experience coordinating or managing complex care packages
• Knowledge of neurological conditions, physical disabilities or long-term complex healthcare needs
• Experience supervising and supporting care teams
• Good understanding of CQC regulations and safeguarding
• Strong organisational, communication and leadership skills
• Full UK driving licence and access to your own vehicle
